How Lifestyle Medicine Maine is different...
There just isn't enough time in the clinic to give you the knowledge and support that you need to radically change your health. It's one thing to tell you what to do, but it's a totally different beast to teach you how to actually implement those changes. Knowing "the why" behind what we do will empower you to critically assess your diet and lifestyle choices so that you can make the best decisions for yourself.

Now, I'm not going to bore you with lectures, but I will challenge you to explore what you know and help you actually put that knowledge into action. Having support and guidance when you've struggled or failed in the past to do what you know that you should be doing is crucial. I'll help you to prove to yourself that you aren't a failure and teach you what is really to blame for not reaching you goal.

Jessica Krol, FNP, DipACLM is available through Lifestyle Medicine Maine to evaluate your health status and to recommend and coach you through a therapeutic lifestyle plan to help you achieve your desired health outcomes. Together, we will determine a concrete plan with measurable goals, and implement therapeutic lifestyle changes with the goal of disease remission.
Health parameters will focus on nutrition, exercise, stress management, sleep and avoidance of risky substances (tobacco and alcohol). Jessica Krol, FNP, DipACLM will be acting not as your primary care provider (PCP) but as a consultant and will partner with your primary care clinician. Consultations are delivered in-person in the clinic and/or via Tele-Health using a HIPPA approved platform and are limited exclusively to Maine state residents.

Is the cost of lab work included as part of the live Group Program?Participants are responsible for the cost of lab work ordered through Rupa Health, which are separate from program fees. All efforts are made to provide transparent pricing that are available to you prior to purchase. Lifestyle Medicine Maine does not charge an additional phlebotomy fee (as this is included in the membership cost). If choosing an outside lab or phlebotomist, you will be responsible for the associated fees.

Can Lifestyle Medicine Maine order/draw lab work and continuous glucose meters (CGMs)?Yes! Jessica Krol, FNP DipACLM can order lab work, continuous glucose monitors (CGMs) as appropriate based on your medical needs. Lab work is ordered through Rupa Health (with pricing fully transparent to you, provided at cost for active members). Some tests can be performed as a blood spot, meaning that you can complete them in the comfort of your own home. Other tests require phlebotomy (lab draw). Lifestyle Medicine Maine offers phlebotomy in the clinic to active program members at no additional cost, in addition to at-home phlebotomy services (for an additional fee) to clients located within 30 minutes of Portland, Maine (learn more). Participants do have the option to choose a phlebotomist of their choice other than our clinician. If choosing a phlebotomist other than Jessica Krol at Lifestyle Medicine Maine, then expect to pay a service or facility fee.

Can I page our clinician if I have a question after-hours or between visits?For non-urgent questions, please use the Practice Better Patient Portal to message Jessica Krol, FNP, DipACLM. Message responses will occur during scheduled business hours. If you would like to schedule a last-minute Telehealth appointment, then please do so. Visits may be scheduled up to 12 hours before the available appointment time. You are welcome to call the office at any time, our clinic number is +1-207-200-7319.

What is your cancellation policy for Weight Loss and Wellness Programs and Subscriptions?The Self Guided Program: This program includes a free 14 day trial, after that you will be charged the agreed upon amount. If you choose monthly billing, then you may cancel at any time. Please note that monthly payments will not be refunded once charge, so do make sure to cancel before your next monthly billing date to avoid being charged. Billing starts on the day of the month that your specific membership is purchased and recurs at 09:00am EST. For example: If your membership fees are paid on the 5th of the month, please cancel before 09:00am EST on the 5th to avoid being charged. ​ The Live Group Program: You may request to leave the Group Program at any point. Requests to stop the program should be submitted online via this form (complete the form here) or you can make this request through Practice Better by messaging the Clinician (Jessica Krol, FNP, DipACLM). If you have prepaid for the program, you will receive reimbursement for the remaining weeks of the program, starting on the day that the request to stop the program was made. For example: If you request to stop the program during Week 06 of the Session, you will receive a refund for the remaining 6 weeks. For clients who sign-up for ongoing Monthly Membership fees as part of the Live Group Program: Cancel at any time without any additional charge. Note that refunds will not be made once billing has been completed. Please remember to cancel before your next billing date to avoid being charged. Billing starts on the day of the month that your specific membership is purchased and recurs at 09:00am EST. For example: If your membership fees are paid on the 5th of the month, please cancel before 09:00am EST on the 5th to avoid being charged. What does it matter what state I am in during my appointment?Our Lifestyle Medicine Clinician, Jessica Krol, FNP, DipACLM is licensed as an independent nurse practitioner in the state of Maine. The law requires patients to be located in the nurse practitioner's state of license. With Telehealth, just like driving across state lines to visit a provider in a physical location, you do not have to live in the state where you receive treatment, but you must be present in that state during your appointment.

What mobile Apps does the Weight Loss and Wellness Program use?Lifestyle Medicine Maine uses a variety of app, depending on which program option your choose... The Self Guided Program: This program uses the Lifestyle Medicine EATs App and the Lifestyle Medicine Maine App. The Live Group Program: This program uses the Lifestyle Medicine EATs App and the Lifestyle Medicine Maine App. As this is a medically facilitated program, we also use the Practice Better Patient Portal as a HIPPA secure portal. Can I use insurance?Lifestyle Medicine Maine does not currently take insurance. We do this so that we can provide participants with unlimited support throughout the program. Participants who join the live Weight Loss and Wellness Group Program with qualifying conditions and commercial insurance may be eligible to use HSA funds. Search "HSA" on the blog to learn more about this.

Does Jessica Krol, FNP, DipACLM work with pediatrics?No. You must be 18 years of age to work with Jessica Krol, FNP, DipACLM through Lifestyle Medicine Maine.

Does our clinician monitor my Practice Better Patient Portal Journal Entries?If you decide to use these patient tools, Jessica Krol, FNP, DipACLM will review them with you at your scheduled appointments with you. Jessica Krol, FNP, DipACLM does not monitor these entries between visits and it is your responsibility to contact Jessica Krol, FNP, DipACLM if you have any questions or concerns about your health in between visits.

What is your cancellation policy for scheduled appointments?Please cancel and/or reschedule your visit at least 24 hours prior to the scheduled appointment by clicking the link in the e-mail reminder or through the Lifestyle Medicine Practice Better Patient Portal. Visits requests for rescheduling the day of your scheduled appointment will not be granted. Missed or canceled visits will be forfeited and are not refundable. This policy applies to new patients, active members and to individually scheduled appointments. ​ For example: If your visit is on a Friday at 2 pm, please cancel or reschedule by Thursday at 2 pm.

What do I do if I have an emergency?Jessica Krol, FNP, DipACLM is a lifestyle medicine specialist and consultant and does not replace your relationship with your primary care provider (PCP). IF YOU HAVE A MEDICAL EMERGENCY, IMMEDIATELY GO TO AN EMERGENCY DEPARTMENT OR DIAL 911.

Do I have to completely change my dietary pattern or can I make small changes?Yes! We will work together to better understand where you currently are on the dietary spectrum and we'll work to make changes that you feel are sustainable.

Can I use my insurance to pay for the Weight Loss and Wellness Program?Lifestyle Medicine Maine is a cash-pay service. We do not currently accept insurance. You are welcome to use your Healthcare Spending Account (HSA) or Flexible Spending Account (FSA) to pay for your appointment if your insurance allows. For patients with private, non-Medicare insurance, we can provide you with "superbill". HSA/FSA eligibility varies by commercial insurance provider. Lifestyle Medicine Maine does not guarantee that HSA/FSA funds can be used. Check with your personal insurance provider. ​ Medicare and Medicare part B patients are unfortunately not eligible to use HSA/FSA funds because the provider at Lifestyle Medicine Maine has had to opt-out of the Medicare system. Can I use my Health Savings Account (HSA) to pay for lab work?If we have commercial insurance, then the most likely answer is 'yes'. If we have government insurance like MaineCare, Medicare or Medicaid, then the answer is likely 'no'. I am sorry, don't yell at me, I don't make the rules.

Can I use my HSA to pay for the Weight Loss and Wellness Program?It depends on your personal health history and which program you are choosing. Let me explain... The live Group Program: participants who have a BMI of 30 or above, or a BMI of 27 and above with a weight-related medical condition. This program is exclusively for Maine State Residents and requires an appointment with our clinician (in person or via tele-health). The Self Guided Program: This program is not eligible to use HSA funds for at the moment. This program is available to all US Residents and does not include medical services or coaching support.

Is the cost of weight loss medication included with the Live Group Program?No. You are responsible for the cost of medication and may be eligible to use your insurance and/or HSA funds to pay for this. Please contact your insurance provider to see if you are eligible or speak to our clinician if you would like to discuss this further.

How often do I need to be seen in the clinic for weight loss medication?All patients are required to attend an in-person appointment in the clinic to determine if weight loss medication is appropriate for you and your personal health history. If we decide that weight loss medication is appropriate for you, then Lifestyle Medicine Maine requires an in-person office visit every 3 months for the first 6 months of treatment. After that, our clinician requires an in-person visit every 6 months for an ongoing prescription. Lifestyle Medicine Maine requires active ongoing participation in the Live Weight Loss and Group Wellness Program for patients with an active weight loss medication prescription. Timeline: Initial in-person appointment to decide if weight loss medication is appropriate for you Follow-up in-person appointment at 3 months Follow-up in-person appointment at 6 months Follow-up in-person appointment at 12 months Follow-up in-person appointment at 18 months *Prescription requires ongoing active participation in the Live Group Program.

Am I eligible for weight loss medication?It depends on our current body weight and personal health history. If you join the Live Group Program and are interested in weight loss medication, then please schedule an appointment in the clinic with our clinician.

What happens to my prescription when I end my participation at Lifestyle Medicine Maine?I hope that you are leaving with the tools that you need to maintain your health and wellness! If you are currently prescribed medication by our Lifestyle Medicine Clinician, Jessica Krol, FNP, DipACLM, if it is safe to do so, Jessica Krol, FNP, DipACLM will provide you with a 30 day supply of the weight loss medication prescribed. You are encouraged to speak to your Primary Care Provider (PCP) if you wish to continue on this medication. After 30 days have passed, Jessica Krol, FNP, DipACLM will not continue to prescribe this medication to you. Please plan accordingly.
